Unity Medical and Surgical Hospital Pre-Surgery

Welcome to the Unity Medical and Surgical Hospital (UMSH) pre-surgical process page.  To begin the pre-surgical assessment, we ask that you fill out your medical history online using One Medical Passport.

Once you complete the process, the pre-surgical assessment nurse will be able to access the information you provided to organize and document your complete medical history to prepare for your surgery.

To begin the online pre-surgical assessment, please review the following instructions carefully before clicking on the link to One Medical Passport.

0. On the One Medical Passport Patient Admissions page, choose whether you are a new patient or a returning patient and follow the instructions to enter the site.
0. As a new patient, you will click on the Enter button and go to the Getting Started page.
0. Once there, please read the instructions and then click on the Next button.
0. You are now on the Facility/Physician Office Information page, which should identify the medical facility as “Unity Medical and Surgical Hospital (Mishawaka, Indiana)”.  Please read the instructions and check the boxes.  After checking the boxes, click on Next to go to the Patient Information page.  Enter the information as instructed on each successive page to complete the process.

Please be sure to have the following information available before you start to create your Medical Passport:

• Health insurance information
• Names, addresses and phone numbers of your physicians
• List of all medications your are taking, the dosage and frequency
• List of all surgical procedures you have ever had and their approximate dates

Please note:  If you are not able to complete your history online, please call the preop nurse between 9:00 a.m. and 5:00 p.m., Eastern Time, at 574-231-6836 as soon as possible to complete your health history.  You will need all of the information described above for this call, which will take approximately 20 to 30 minutes.
